What if I do not have all the information required on the Veteran form?

The form must be completed and payment must accompany the form to submit. You may not submit a partial form and complete it at a later time unless you wish to have some information withheld from the paver.

If you would like to have information withheld from a paver and leave a line or part of a line of the Veteran paver blank, simply draw a line through that area on the form and initial it.

Yes. A separate form is required for each paver. To keep the forms together and to aid in our processing and placement of the pavers in the Memorial, please use the same purchaser name for each paver when purchasing multiple orders. You may write one check to cover the total cost of the pavers. Please staple the forms together prior to mailing or when received at the HOA/Council Office. Paper clip the check to the forms.

How will the pavers be placed? May I ask for a special location in the Circle of Honor?

The pavers will be placed in the order in which they are received, starting at The Great Seal and working in a circle from The Great Seal and out from that point. Due to the number of bricks in the Circle of Honor, we cannot guarantee any special placement of your paver. We will make every effort to keep family pavers together, as long as the same purchaser is provided on each of the multiple forms.
